Our Specialist Support Coordinators assist people across Australia to discharge from hospital & aged care facilities into suitable long-term accommodation, complete with the necessary care and equipment.

Our i’ara Hospital to Home (H2H) program offers a FREE service to patients in hospitals nationwide and was developed after our team observed multiple hospital discharges. Recognising the need for a more streamlined process that leads to faster discharge times, better funding outcomes and a decrease in hospital re-admissions.

Navigating the hospital system, the NDIS or the various state injury schemes can be complex and long. Rest assured that working with our specialists we will be with you every step of the way and we will shave months off your hospital stay.

To be eligible for our Hospital to Home service you must:

• Be eligible for NDIS funding
• Be living in a hospital or aged care facility at the time of referral.

• You, your family & friends
• Hospital Staff – Social Workers, Doctors, Allied Health Professionals, Nurse Navigators

Let’s get things moving

With your permission we will speak to your treating team at the hospital (Social Workers, Allied Health, Doctors) and the NDIA. We will work closely with everyone to prepare your applications for equipment, housing and care support ready for discharge.

If you are under 65 years of age and living in an Aged Care Facility – please contact our team as we will be able to work with you to find you more suitable housing and support.

How we can help

Before discharge, our team will connect you with the most suitable care supports in your local area to support you at home. We will work with these support providers to complete in-hospital training so you feel comfortable and confident before leaving.

Examples of supports that can be organised include:

• In-home care (showering, toileting, feeding, cleaning, supports)
• Nursing
• Physio, Occupational Therapy, Speech Pathology
• Equipment (wheelchair, bed, hoist etc)
